The nickel fact almost nobody states plainly
"Surgical steel" sounds like it should be the safest thing available. The name is doing a lot of work.
316L stainless steel ā the grade most body jewellery is made from ā is an austenitic steel, and nickel is what makes it austenitic. It typically contains 10% to 14% nickel by mass. The metal is not defective; nickel is a deliberate, structural part of the alloy.
The L in 316L stands for low carbon. It refers to carbon content, which affects corrosion resistance at welds. It has nothing to do with nickel, and it is routinely misread as though it did.
Implant-grade titanium to ASTM F136 is Ti-6Al-4V ELI: titanium, aluminium and vanadium. There is no nickel in the specification.
So is surgical steel unsafe?
Not inherently, and it would be dishonest to say so. Implant-grade steel to ASTM F138 is on the accepted materials list for piercings in jurisdictions that regulate this, and plenty of people wear steel for years without any reaction.
The nickel in a properly finished, high-quality steel alloy is bound in the metal structure and releases very slowly. The UK and EU set the legal threshold at less than 0.2 µg/cm² per week for jewellery in a pierced body part, and good steel can meet it.
Two situations change the calculation. First, if you already react to nickel, steel is a risk you do not need to take. Second, cheap steel with no standard number behind it may not be 316L at all, and may not meet the release limit ā and you have no way to check.

What the price difference actually buys
At UK retail, a titanium flat back stud typically sits a few pounds above a steel equivalent ā often the difference between roughly Ā£8āĀ£12 for steel and Ā£14āĀ£22 for implant-grade titanium.
For that difference you are buying three things: an alloy with no nickel in the specification, a standard number you can ask a seller to evidence, and ā usually, though not automatically ā better finishing, because suppliers who bother with certified titanium tend also to bother with internal threading and a properly polished surface.
Titanium is also around 40% lighter than steel of the same volume. On a long dangle or a large hoop that is genuinely noticeable; on a small stud it is not.
When the difference matters most
It matters most for a fresh or healing piercing, for anyone with known nickel sensitivity, for placements worn continuously such as a navel bar against a waistband, and for anyone who has previously reacted to costume jewellery.
It matters least for a fully healed lobe worn occasionally by someone who has never reacted to anything. In that case the honest answer is that good steel is fine, and anyone telling you otherwise is selling.
