Short answer: yes, if the jewellery is the right type. Flat back studs are designed to be slept in β the smooth disc sits against the back of the ear with nothing to dig into your head. Butterfly backs and hoops are not, and sleeping in them is one of the most common causes of irritation bumps on cartilage piercings.
For a healing piercing you have no choice: the jewellery stays in. The question is whether it is the right jewellery.

The difference matters most on cartilage. A butterfly clasp pressed between your ear and a pillow for eight hours transfers that pressure directly into a healing channel β which is how irritation bumps form.
Why bar length matters as much as the back
A flat back stud that is too long still moves and catches. One that is too short presses into tissue.
Fresh piercings are fitted long to allow for swelling, then downsized once that settles β usually from 8mm to 6mm on a helix. If your stud spins freely or snags on a jumper, it is too long for your current stage of healing.

Can you shower with them in?
Yes. Water is fine β in fact a healing piercing should be rinsed in the shower. What matters is what else runs over it. Shampoo, conditioner and body wash leave residue in the channel, so rinse thoroughly at the end.
Titanium does not rust or tarnish in water, unlike silver or plated pieces.
Can you swim with them in?
Not while healing. Pools, hot tubs, lakes and the sea all carry bacteria that a fresh channel cannot defend against, and chlorine irritates healing tissue.

How to sleep more comfortably
- Sleep on the opposite side while a piercing is healing
- Use a travel pillow and rest your ear in the hole
- Change your pillowcase regularly
- Keep hair tied back so it does not wrap around the jewellery
- Downsize to the correct bar length once swelling settles
The metal matters too
